Psychosocial Rehabilitation Facilitator (QP)

Job Description: 

Meridian Behavioral Health Services is seeking a Psycho-Social Rehabilitation Facilitator in Haywood County.  This position is responsible for implementing, organizing and managing psycho-social rehabilitation for severely and persistently mentally ill adults.  The PSR Facilitator position will work in conjunction with PSR Coordinator to develop and maintain educational/rehabilitative curriculum for the psychosocial rehabilitation program, recruit, interview, assess and develop treatment plans for potential PSR clients, as well as having awareness of budgetary goals, and working to meet those goals.

Additionally, this position will:

  • assist in maintaining necessary supplies for program activities, including meals,
  • organize and conduct offsite activities,
  • maintain frequent contact with potential referral resources, and other parties and organizations in the community, which will interact with the PSR, including Vocational Rehabilitation, DSS, public transportation agencies, and others,
  • assist in creating and maintaining a therapeutic/rehabilitative environment for program participants,
  • be responsible for overseeing the transportation of clients to and from the program when this service is available, as well as transportation for outings,
  • facilitate conflict resolution and crisis intervention with program clients to facilitate the therapeutic environment,
  • report regularly to PSR Coordinator about program status, attendance at the program and critical incidents as they occur,
  • consult with PSR Coordinator with problem areas or as needed to obtain services, material, and deal with emergency situations,
  • be accountable for the timely accomplishment of treatment goals, including the appropriate use of crisis intervention and emergency management skills when clinical issues of an emergent nature arise,
  • provide written, telephone and face to face consultation services to various persons and organizations which assist the client. These include schools, courts, probation officers, hospitals, medical care providers, law enforcement officers, social services and community agencies,
  • be responsible for the timely recording of all treatment and clinically significant events for the medical record in the manner prescribed by the agency quality assurance and quality improvement manuals. He/she is further required to record information in activity logs, and on various billing forms, and
  • complete various utilization, reporting, and monitoring forms for insurance companies, preferred provider networks, health maintenance organizations, managed care entities, and other third-party payers.

The applicant for this position must be a Qualified Mental Health Professional with a Bachelor's Degree from an accredited university in human service field and at least two years of full-time accumulated supervised experience with the population served.

Applicants must also have:

  • a valid driver's license without violations or restrictions which could prevent completing all required job functions,
  • reliable transportation, and
  • and moderate computer skills.
