Nurse (RN) - Assertive Community Treatment Team

Job Description: 

Meridian Behavioral Health Services is seeking a RN or LPN to join our Assertive Community Treatment Team (ACTT).  ACTT is an evidence-based wrap-around model of care that supports individuals with severe and persistent mental illness to improve their overall well-being while remaining in community living.  The ACTT Nurse is responsible for assessing physical needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist. The ACTT nurse is also responsible for providing a range of treatment, rehabilitation and support services including general and condition-specific health education, medication education, and psychiatric rehabilitation. Being part of the "on call" rotation is an expectation. One year of Psychiatric experience strongly desired.

Applicants must have:

  • a current RN or LPN license in NC,
  • a valid driver's license without violations or restrictions which could prevent completing all required job functions,
  • reliable transportation, and flexibility and moderate computer skills.
